If you indicate on the inquiry form that you would like to access Assumption’s transfer application, you will receive an invitation to create an account in our secure Blackbaud Admissions Portal.
Once you create an account within the Blackbaud Admissions Portal, you may:
- access and complete the transfer application.
- save application progress.
- check the status of your student’s application.
- communicate with the Admissions Office.
You will need the following items to complete the transfer application:
- unofficial transcript, official transcript, or past report cards and current grades-to-date
- attendance information, along with placement test scores or any standardized test scores (PreACT, ACT, SAT, MAP, etc.)
- information from the parent(s)/guardian(s) and information from the student as to the reason she wishes to transfer to Assumption High School, providing as much detail as possible

Please note if you are applying for a transfer to Assumption and would like to be considered for financial assistance, you must complete the FACTS application. The deadline to complete FACTS is January 15, 2026 for the 2026-27 school year. Late applications will be considered but are deferred until all applications filed by the deadline are reviewed and awards granted.
